A: You should notify Capita Registrars in writing. If you hold shares in joint names, the notification to change address must be signed by the first named shareholder.

This is to protect Healthcare Locums plc from possible misuse of your certificate while it is not in your possession. A replacement certificate will then be sent to you. There is a charge for this service, so we advise you to keep your share certificates in a safe and secure place.

A: If shares are bought at different times the registration details given to Capita Registrars might differ slightly on each occasion (e.g. Mr John P Smith or Mr John Smith). The shares can then be sold by the personal representative or a stock transfer form can be used so that the shares are registered in the name of the deceased's beneficiary. If the shares were held jointly, Capita Registrars will remove the deceased's name from the share register on receipt of a copy of the death certificate. If you cannot appoint a proxy to attend the meeting in person, you can still send the proxy form and in the event of a poll on any resolution, the Chairman will cast your vote for you in line with the instructions you give on the form.
